If you are an Active Indemnity PPO Medical Plan participant, the Fund offers you and/or your enrolled spouse/partner opportunities to earn wellness incentives. When you do, you’ll have more cash in your HRA and a better understanding of how to manage your health. The purpose of the HRA is to help you pay less of your own money—saving you potentially hundreds of dollars—for health care. That’s why the Fund strongly encourages you to understand how the HRA works.

If you’re an Active Indemnity PPO Medical Plan participant, you and/or your enrolled spouse can both complete activities to earn HRA money for maximum funding based on your coverage category. Bump up the balance in your Health Retirement Account (HRA)—and pay less out-of-pocket—by completing any of the following 7 healthy activities.
Gold/Silver participants earn $125 per activity; Platinum/Platinum Plus participants earn $150 per activity.
- Option 1: Establish your personalized Benefits Plus Portal account. Get started here, and follow the prompts to register.*
OR
- Option 2: Provide the Fund with up-to-date contact information for you and your primary doctor.* You must return a Contact Info Form to the Fund Office to complete this activity.*
- Option 3: Complete a Health Risk Questionnaire (HRQ). Do your HRQ online: takeyourhrq.com or by phone: 888-901-0477.
- Option 4: View an online health benefits video. Access the video at scufcwfundslearning.com.
- Option 5: Get an annual physical. This activity is processed automatically through the Fund’s claims system once completed.
- Option 6: Get a health screening. There are two ways to complete this activity:
- Do your screening with your doctor and return this form to the Fund Office. OR
- Do your screening through a participating in-store pharmacy. Currently, you may schedule an appointment through an Albertsons/Vons participating pharmacy.
- To find a participating Albertsons or Vons location, go to https://kordinator.net/wellconnected and select “UFCW Biometric Screening” from the pull-down menu. Follow the prompts to schedule the screening.
- Option 7: Get certain preventive health care services, including a flu shot. In most cases, your service(s) are processed automatically through the Fund’s claims system. However, sometimes this form may be required.
- Option 8: Do a healthy lifestyle activity, such as participating in a smoking cessation program or a weight management program for three consecutive months. Use this form to report your lifestyle activities; additional documentation may be required as noted on the form.
*Note: Once you have registered for your portal account (Option 1), you will not be eligible to receive funding for this Healthy Activity in following years. It’s a “one-time” activity. Also, the plan allows you to receive HRA funding for registering your portal account (Option 1) OR submitting your up-to-date contact information (Option 2) during a given HRA funding cycle (June 1 to May 31). You cannot receive credit for both activities.
Disease Management
You may be eligible for the Disease Management Program if you are identified as suffering from coronary artery disease (CAD), asthma, or diabetes. This program is designed to help prevent or minimize the effects of these diseases.
If you are eligible, you’ll receive a letter from the Fund indicating that the Disease Management program administrator will call to let you know whether you qualify. If you do qualify, once enrolled and actively participating, you will be entitled to lower prescription drug copays for most of the medications for your condition, as well as additional contributions to your HRA.
If you are invited and decide not to participate, your annual deductible will go up.
