Whether you need prescription eyeglasses or just an eye exam, the Fund’s vision coverage will help you pay for covered vision services for you and your enrolled dependents.
Vision benefits are automatically included with medical coverage at no additional cost to you. You may opt-out by calling the Fund Office and completing the proper form. Dropping your vision coverage will not reduce your weekly payroll deductions.
You may use any licensed vision care provider for exams and corrective eyeglasses or contact lenses. Benefits for corrective lenses and frames are payable only if no more than 12 months have elapsed between the date of the last vision examination and the date glasses or contact lenses are ordered. (This rule does not apply when a lens change is required following eye surgery or other conditions.)
You pay any charges that exceed the maximum annual benefit shown in your plan’s Benefits Chart.
