Complete your HRQ in one of three ways:
1. Online: www.2013HRQ.com
2. Phone: Call HMC toll-free at 888-901-0477
3. Paper: Contact your Union Local, the Fund Office, or HMC to request a paper HRQ and postage-paid return envelope.
REMEMBER: Completing the HRQ is just one of many Healthy Activities you can do to get Earned HRA Contributions. To receive full funding for your 2013 HRA, you must complete all of your activities by May 31, 2013. Save Money with the Market Priced Drug Program
The MPD program helps you and your doctor identify lower cost prescription drugs for treating some very common health conditions.
Lower cost drugs are called "Preferred" Drugs under the MPD program. When you use a Preferred Drug to treat a condition included in the MPD program, you will pay your current copayment. However, if you use a drug that is not on the Preferred Drug list (known as a "Non-Preferred" Drug), your out-of-pocket cost will be much higher.
